The discharge process at ESIC Hospital is designed to ensure that patients leave the hospital safely and with all necessary information for continued care. Here’s a step-by-step guide to help you understand what to expect during discharge:

1. Discharge Planning

Location: Patient Bed / Discharge Planning Desk
Process:

Medical Review: The medical team will review your condition and treatment progress to determine if you are ready for discharge.
Discharge Criteria: Once you meet the discharge criteria, a discharge plan will be created, which includes instructions for follow-up care, medications, and any lifestyle changes needed.

2. Preparation for Discharge

Location: Patient Bed/Ward
Process: 

At the time of discharge, medication is processed online & patients collect it from the Pharmacy counter

Discharge Summary: A discharge summary will be prepared, detailing your hospital stay, diagnosis, treatments received, and instructions for follow-up care.
Medication Instructions: The medical team will provide you with prescriptions for any medications you need to take after leaving the hospital, along with instructions on how and when to take them.
Follow-Up Appointments: Schedule any necessary follow-up appointments with your healthcare provider or specialists.

3. Patient Instructions

Location: Patient Room / Discharge Desk
Process:

Care Instructions: Review and follow the discharge instructions provided by your healthcare team. This includes guidance on wound care, activity restrictions, diet, and medication management.
Emergency Contact: Know whom to contact if you experience any complications or have questions after discharge.
Patient Assistance: If you require additional support during the discharge process, such as help with understanding your discharge instructions or arranging transportation, patient assistance services are available.
Patient Feedback: You can provide feedback on your hospital experience through a feedback form or online portal, which helps us improve our services.
